March 14, 2011 <br />Thuy Tran <br />Dear Ms. Tran: <br />In accordance with violation CCR 2711(b) and (c), that form has been submitted to EHD. I am <br />not sure about the date it was submitted but I know that it was. HSC 25292.2 and CCR 2632 (b) <br />has been sent to your office on 2/11/2011. CCR 2635 spill container test by Epic Compliance <br />was performed and initially, they did not do it properly. Later, it was corrected and Michelle <br />Henry approved and was within compliance. CCR 2715 regarding designated operator was <br />resolved. I understand that Brian Dunahay performed the monthly inspection for 1/2009 and <br />though he was regarded as a non -designated operator, we had Steve Zwahlen from Franzen -Hill <br />perform the task.
