Annual monitoring system certification testing was last performed on 11/19/09 and was due by <br />11/2010. An monitoring system certification test started 02/07/11 but was incomplete - 87 annular was <br />not able to be removed from interstitial space to be tested since the protective outer sheath covering of <br />the black and red wires was in poor condition and frayed. The wiring was exposed and pulling the <br />annular sensor out would cause tension and possible breakage. The testing of the 87 annular sensor is <br />completed today. Outer sheath wiring was repaired with black electrical tape. Annular passed <br />functionality test. Spill buckets and leak detectors were complete 02/07/11. These tests are required <br />once every 12 months. Be aware that today's follow up inspection will be charged at our current hourly <br />rate. Repeat violation. <br />Submit to SJC EHD within 30 days (03/11/11): <br />1. one copy of the completed "Return to Compliance Certification" form <br />2. a written corrective action statement for the above violation <br />Notes: <br />- Secondary containment next tri -annual testing next due by 06/2011. Provide EHD 48 hours notification <br />of the scheduled test. <br />- Received Spill bucket test results from 02/07/11 today.
