This permit is only approved to determine what repairs <br /> need to be made. This permit is not to make any repairs until the following conditions are met: <br /> Conditions of approval: <br /> 1. Contact the local fire department, local planning or building department and the San <br /> Joaquin Valley Air Pollution Control District for any applicable permits. <br /> 2. Any changes shall be submitted to the San Joaquin County Environmental Health <br /> Department(EHD) for approval prior to commencing work.All new addenda supercede <br /> the previous ones. <br /> 3. Send an updated scope of work to the EHD,after breaking concrete and exposing <br /> the tank system,with the parts needed for the repair. Include cut sheets and UL <br /> listings for each part. <br /> 4. If concrete is being broken near the UST,pipeline and tank secondary containment <br /> testing must be performed in accordance with the manufacturer's guidelines. Testing <br /> must be done after returning the facility to operating condition. Results must be received <br /> by the EHD within seven days of returning the facility to operating condition. <br /> 5. All components must be certified by an independent testing organization and labeled as <br /> required by the organization. (UL,ULC,ANSI,API,ASTM,etc. See California Code of <br /> Regulations(CCR),Title 23 Chapter 16 Table B for complete list). The component must <br /> bear the appropriate mark as required by the independent testing organization. <br /> 6. Qualified service technicians and installers must possess or work under the direct and <br /> personal supervision of an individual physically present at the work site who possesses <br /> all of the following: <br /> a. One of several permissible valid,current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company),and <br /> b. All valid,current California UST certificates issued by the International Code <br /> Council(ICC),that are applicable to the work being performed,and <br /> c. A valid, current certificate of training from the manufacturer(s)of the <br /> component(s)being tested,repaired,or serviced; <br /> Page 1 of 2 <br />
