Provide a complete application in triplicate on the updated retrofit or piping repair permit <br /> (Copy enclosed). <br /> 2. Provide copies of all manufacturer's training certificates,ICC certifications,contractor's <br /> license,etc. for individuals who will be performing the work(Question 2 on application). <br /> 3. Provide a detailed scope of work, including how the repairs will be made,and 3`d party <br /> approval for repairs. <br /> 4. Sampling is required, 1 sample�per 20 lineal feet. Complete sampling information in the <br /> permit application, including 3 party sampler,sampling protocol,and authorization to <br /> release. <br /> 5. Describe how the excavated soil will be handled. <br /> 6.
