Ensure a copy of the"Site Health and Safety <br /> Plan" is available on the jobsite as required by Title 8. <br /> 3. Detailed description of work to be completed. List components to be repaired or replaced and attach a diagram drawn <br /> to scale showing location of repairs and/or replacements. If repairing a component,describe how this will be done. (if <br /> adding piping, UDC's, or other UST equipment, or performing tank top upgrade, use the UST Installation Application <br /> pages 4-8 as necessary for a timely plan review): <br /> (1) USA Site. (2) Sawcut. demo, remove and dispose of 41W x 8'L x 6"thick concrete <br /> pad. (3)Excavate 4'6" deep hole. (4) Expose cracked secondary piping. (5) Make <br /> repairs to fiberglass piping. Conduct air test while repaired pipe is exposed. <br /> (6)Backfill and replace concrete 6" deep with #4 rebar on 8" center each way. <br /> (7) Conduct third party testing of lines and tank, and lake testing of sump. <br /> NOTE: See attached diagram showing location of repairs. <br /> 4.
