September 15, 2016 <br />San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Ave <br />Stockton, CA 95205 <br />IWO <br />LIQUID:: <br />RECEIVED <br />A <br />DEPARTMENT <br />RE: Response to Violations for SJ General Hospital, Inspection Date August 25, 2016; Report <br />received August 30, 2016 <br />Il[GAIS :[e]JIIaLT4 fflKelm <br />Written corrective action statement for the violation as stated below: <br />Worked with the facility to update the California Environmental Reporting System (CERS) in <br />regards to the following violations. Please note the CERSID: 10643278 <br />Item 105 - CCR 2632(d) - Failed to have an approved UST monitoring plan. Response: CERS <br />will be updated. <br />Item 107 - CCR 2532(d)(1)(C) Plot plan/site map not submitted or failed to completely show <br />where monitoring is performed. Response: Site Map has been revised and will be uploaded to <br />CERS. <br />Item 201 - CCR 2713(b) Failed to maintain alarm logs and/or records of follow up actions. <br />Response: Alarm logs and printouts will be stored in the binder for 3 years and kept on site. <br />Attached are the following records which have been placed in the binder on site: 1. date and time <br />of all monitoring or sampling; 2. monitoring equipment calibration and maintenance records; 3. <br />results of any visual observations; 4. results of sample analysis performed by a lab or in the field; <br />5. logs of readings of gauges or other monitoring equipment, ground water elevations, or other <br />test results; 6. results of inventory readings and reconciliations. <br />Item 206 - CCR 2712 Failure to comply with UST operating permit conditions. Response: A <br />copy of the UST operating permit was in the binder under the response plan section. <br />Item 209 - CCR 2715(c) Designated operator did not inspect all required items and/or inspection <br />reports not completed. We had an issue with the system dates; this issue has been resolved. Our <br />printer wasn't working properly and therefore, we were pulling reports on different days. We've <br />purchased a new printer just for this purpose.
