The site plan did not include the location of the <br />annular sensor. A site plan must be submitted identifying the locations where monitoring will be performed. <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload <br />a copy of the UST Monitoring Site Plan. <br />This is a minor violation. <br />201 <br />CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br />Complete, accurate maintenance and monitoring records for the last three years were not found on site. No work <br />orders were found on site. An invoice dated 1/28/16 indicated a technician was on site on 1/21/16 in response to a <br />sump alarm, stating liquid was removed from the sump and the alarm was cleared by the technician. The alarm logs <br />and alarm printouts maintained by the facility state the alarm was cleared on 1/20/16. These records shall be <br />maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or <br />sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) <br />results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring <br />equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. <br />Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and <br />submit copies to the EHD. <br />This is a minor violation. <br />206 <br />CCR 2712 Failure to comply with UST operating permit conditions. <br />A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all attachments <br />and conditions shall be retained at the facility.
