Escotto <br />Subject: California Environmental Reporting System Uploads <br />Good morning Jesse, <br />I called you earlier today and left a voice message concerning the underground storage tank (UST) submittals to the <br />California Environmental Reporting System (CERS) for San Joaquin General Hospital at 500 West Hospital Road, French <br />Camp, California. <br />Submittals were not accepted on October 19, 2015, by the San Joaquin County Environmental Health Department (EHD). <br />Specifically, the following revisions are needed: <br />• The Business Activities submittal does not indicate that there are aboveground tanks at the facility. See <br />attachment for clarification. <br />• The Facility Summary for CERS ID:10643278 lists Jesse Escotto as the owner; change Owner Information to San <br />Joaquin County. See attachments. <br />• UST Facility Operating Permit Application lists the Financial Responsibility Mechanism as Self-insured; change <br />this to "No". See attachments. <br />• UST Tank Information indicates that there are no spill buckets or striker plates; change these to "Yes". See <br />attachments. <br />
