On site alarm records indicated water was removed <br />from a sump on 4/30/18, tank cleaning was conducted on 1/3/18, and sensor repairs were conducted on 8/30/17. <br />Alarms occurred on all of these dates. No work orders were found. These records shall be maintained on site for at <br />least three years. <br />-Annual primary line tightness test results for 2016 and 2017 were not found on site. <br />Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and <br />maintenance records-, (3) results of any visual observations; (4) results of sample analysis performed a lab or in the <br />field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) <br />results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring <br />records for the last three years on site and submit copies to the EHD. <br />This is a repeat violation, Class 11. <br />206 <br />CCR 2712 Failure to comply with UST operating permit conditions. <br />A copy of the 2018 UST operating permit was not found on site. A copy of the UST operating permit and all <br />attachments and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it on <br />This is a repeat violation, Class 11. <br />209 <br />CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to check and note the correct test dates for the annual line tightness testing on all of <br />the designated operator monthly inspection reports. The test date was incorrectly listed as 8/27/15. During the <br />monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system <br />have been completed and document the dates they were done. Ensure that designated operators performing monthly <br />inspections at this facility are including all of the required information on the reports. <br />The designated operator failed to document all the alarms or attach complete alarm histories on the designated <br />operator monthly inspection report and failed to check that the alarms were responded to appropriately. The missing <br />alarms include: <br />-1/3/18 probe alarm. The alarm log maintained by the site states this alarm was caused by tank cleaning. <br />-4130/18. The alarm log maintained by the site states this alarm was cleared when water was removed from the sump. <br />During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br />that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br />documentation taken in response to any alarms to the monthly report.
