This work was completed on August 4, 2017. The permit <br />has been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br />any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br />commencing work. All new addenda supersede the previous ones. <br />3. Functionality of the monitoring system was verified during the 8/21/2017 annual UST <br />inspection. No further inspections are required at this time. <br />4. If concrete will be broken near the 20,000 gallon diesel underground storage tank, a secondary <br />containment test must be performed on the affected UST once concrete is poured in <br />accordance with the manufacture's guidelines. <br />5. Any individual performing the work of a service technician must meet the following <br />qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible valid, <br />current contractor's licenses issued by the Contractors State License Board (may be issued <br />to the installation company), or a Tank Tester's license issued to the individual by the State <br />Water Board, and <br />b. Possess or work under the direct and personal supervision of an individual physically <br />present at the work site who possesses a valid, current California UST Service Technician <br />certificate issued by the International Code Council (ICC), and <br />c. Possess a valid, current certificate of training from the manufacture(s) of the component(s) <br />being tested, repaired, or serviced. <br />6. Individuals installing underground storage tank system components must possess or work <br />under the direct and personal supervision of an individual physically present at the work site <br />who possesses at of the following: <br />a. A valid, current contractor's license issued by the Contractors State License Board, and <br />b. A valid, current UST Installation/Retrofitting certificate issued by the International Code <br />Council (ICC), and <br />1868 E.
