This work was completed on January 23, 2015. The <br />permit has been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br />applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br />commencing work. All new addenda supersede the previous ones. <br />3. Contact Elena Manzo at (209) 953-7699 to arrange the following appointments (a 48-hour notice is <br />required for inspection appointments). Please be aware that inspections for emergency work must be <br />scheduled within five days of permit approval. <br />• Leak detector test: Leak detector must be tested to confirm that the device can detect a leak of 3 <br />gallons per hour at 10 psi. <br />4. If concrete will be broken near the 87 -octane tank, a secondary containment test must be performed <br />on the 87 -octane tank once concrete is poured in accordance with the manufacturer's guidelines. <br />5. Any individual performing the work of a service technician must meet the following qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible valid, current <br />contractor's licenses issued by the Contractors State License Board (may be issued to the <br />installation company), or a Tank Tester's license issued to the individual by the State Water <br />Board, and <br />b. Possess or work under the direct and personal supervision of an individual physically present at <br />the work site who possesses a valid, current California UST Service Technician certificate issued <br />by the International Code Council (ICC), and <br />c. Possesses a valid, current certificate of training from the manufacturer(s) of the component(s) <br />being tested, repaired, or serviced. <br />
