Contact the local fire department, building department and San Joaquin Valley Air <br />Pollution Control District for applicable permits and requirements. <br />2. Any Changes in information or scope of work must be submitted to EHD prior to <br />commencing work. <br />3. A certified installer must be on site at all times during the installation. If the certified <br />installer leaves the premises for any reason, all work must be ceased without exception. <br />Until the certified installer returns on site, work shall not continue. <br />4. Contact Steven C. Shih at (209) 468-9850 to arrange the following inspection <br />appointments; 48-hour notice for inspection appointments is required. <br />a. Corrosion protection: all equipment in contact with backfill must have corrosion <br />protection. <br />b. Overfill prevention must satisfy CCR 2635(b)(2). The measurement of flapper <br />shall be demonstrated. <br />c. Spill bucket must satisfy CCR2635(b)(1) and pass a 30 -minute lake test or <br />equivalent method after concrete was poured. <br />d. A third party certified tank integrity test must be performed and results received <br />by this office within 7 days of completing the project. <br />5. Permit expires within 90 days. Note: if an extension to the expiration date is <br />required, a request must be received in writing to this office prior to the expiration <br />date. <br />Thank you, <br />Steven C.
