One of the construction <br /> workers, stated that several weeks back, the panel was making a lot of noise so they removed the power <br /> connector inside the panel to silence it. Mr. Joe Nunez, the superintendent onsite, indicated that he was <br /> aware that the monitoring panel should be on. While onsite, the power to the Veeder Root was restored (by <br /> the construction worker onsite) and the monitoring panel was "warm started". Several communication-type <br /> alarms were visible on the display upon reconnection. Furthermore, multiple cut wires at and near junction <br /> boxes inside the UDCs, to which Veeder Root sensors are connected, were noted. <br /> CORRECTIVE ACTIONS/ORDER: <br /> An underground tank system that is temporarily taken out of service, shall continue to be subject to <br /> all permit, inspection and monitoring requirements (HSC 25298). Call the EHD to schedule an <br /> inspection as soon as possible (but no later than February 23, 2015) to verify that the UST system is <br /> continuously monitored, as required, or submit an application to the EHD to temporary close the US <br /> system and satisfy all temporary closure requirements.
