The permit has been approved with the <br />following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Elena Manzo at (209) 953-7699 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). <br />a. Sensor functionality: Functionality of the monitoring system for all sensors must be <br />verified to ensure that monitoring has been restored to meet the requirements of Title <br />23 California Code of Regulations and the Health & Safety Code. A copy of the <br />alarm history, printed prior to performing the "cold start" of the Veeder Root <br />system, shall be made available at the time of EHD inspection. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work. site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), or a <br />Tank Tester's license issued to the individual by the State Water Board, and <br />b. A valid, current California UST Service Technician certificate issued by the <br />International Code Council (ICC), and <br />c. A valid, current certificate of training from the manufacturer(s) of the component(s) <br />being tested, repaired, or serviced. <br />
