Monitoring system certification test results for July 2, 2014 was not found onsite. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a minor violation. 208 CCR 2715 Designated operator monthly inspection reports not maintained on site for at least 12 months. The April 2015 designated operator monthly inspection reports were not found on site. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to the EHD. This is a minor violation. 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. The Bravo box float and chain (north) in the UDC 1/2 sump failed to stop the flow of product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician adjusted the chain(s) and verified functionality. This is a minor violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by July 31, 2015. Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if applicable), and the Owner Statement of Designated Operator Compliance. The UST Owner/Operator: Written Agreement, if applicable, can be stored at the facility or uploaded into CERS. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
