Olive Ave. <br />Stockton, CA 95215 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the application for a permit to replace the 87 -octane fill sump spill container with an OPW Spill <br />Container, and a new NVMEM-203 for the existing monitoring panel which requires a cold start <br />on the underground storage tank (UST) system at the above noted address. The permit has <br />been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Jeffrey Wong at (209) 468-0335 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days of permit <br />approval. <br />a. Sensor functionality: Functionality of the monitoring system must be verified to <br />ensure that monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and the Health & Safety Code. A copy of the alarm <br />history shall be printed prior to performing the "cold start" of the Veeder Root system <br />and made available at the time of EHD inspection. <br />b. Spill bucket test: Verify minimum 5 -gallon capacity and pass a one hour hydrostatic <br />test (or equivalent). Proper drainage must also be verified. <br />Please be aware that the facility cannot operate the UST system OR receive fuel in <br />the 87 -octane tank prior to the EHD inspection and approval. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />
