The following are Repeat Class II Violations <br />305, 310, and 313 and Minor Violations - Notice to Comply: 999 <br />305 (Repeat). 91 and 87 -octane Bravo float and chain in Dispenser #1/2 and 3/4 UDC sumps failed to <br />stop the flow of product at the dispenser when tested. All monitoring equipment shall be maintained to <br />activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. <br />The service technician adjusted the chain and verified functionality. Violation was corrected on site. <br />310 (Repeat). Liquid was observed in the 87 and 91 -octane fill sumps and the STP sumps: about 3 <br />cups in the 91 STP sump, 3 cups in the 87 STP sump, 4 cups in the 91 fill sump, and about 2 gallons in <br />the 87 fill sump. If water could enter into the secondary containment by precipitation or infiltration, it <br />must be removed and disposed of properly. Immediately remove this liquid, make a hazardous waste <br />determination per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the <br />sumps are maintained free of liquid. Violation was corrected on site. <br />313 (Repeat). The 87 -octane spill container in a secondary sump without liquid sensor, failed when <br />tested. The technician on site stated the leak was at the swivel adaptor, not because of the integrity of <br />the spill container. The technician attempted to replace the swivel adaptor and gasket, and was unable <br />to complete the task and retest the spill container due to lack of replacement parts. All spill containers <br />shall have a minimum capacity of five gallons and be capable of containing a spill or overfill until it is <br />detected or cleaned up. Immediately submit the result of the annual hydrostatic test after the swivel <br />adaptor has been replaced and the spill container retested by July 13, 2013 (a year from the date when <br />the spill container was last tested) If the hydrostatic test of the 87 -octane spill container still failed after <br />the replacement, immediately discontinue deposition of petroleum into this tank until the component is <br />repaired or replaced by a properly licensed, trained, and certified contractor under permit and inspection <br />of the EHD. If the spill container can't be replaced immediately, there is a possibility that the UST <br />system may be red tagged to prevent fuel inputs.
