WATERLOO RD. <br /> The permit application to expose the vent piping for the exploration of a leak has been <br /> approved with the following conditions: <br /> 1. Contact local Air and Fire agencies for applicable permits. <br /> 2. A complete description of the work performed must be submitted prior to <br /> scheduling the final inspection. <br /> 3. If any portion of the UST system is to be repaired, or replaced, this department <br /> must be notified so that an inspection can be arranged and parts can be approved <br /> prior to commencing work.Per Environmental Health Assessment of findings, <br /> new plans and a permit application may be required. <br /> 1 . Sampling may be required. 7,r'w a o b-,-K �� <br /> The sampler, certified laboratory, and type of samples shall be approved by this <br /> department before sampling occurs. <br /> 5. A tank integrity test may be required by this department,prior to the completion <br /> of this project. Results shall be submitted within seven days of testing. <br /> 6. Arrange a final inspection with this office prior to returning this station to full <br /> service. <br /> 7. Give 48-hour notice for all inspection appointments. <br /> a.
