Waterloo Road <br /> Stockton, CA 95205 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to replace the existing Incon TS-1001 ATG and TS-ROM Relay <br /> Output Module with a new TS-1001 ATG and TS-ROM Relay Output Module, replace the 87, <br /> 91, and Diesel STP relays with Red Jacket 079-425-1 STP relays, and to test sensors to verify <br /> positive shutdown requirement has been met on the underground storage tank (UST) system at <br /> the above noted address. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Jeff Wong at (209) 468-0335 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Sensor functionality: Functionality of the monitoring system must be verified to <br /> ensure that monitoring and positive shutdown has been restored to meet the <br /> requirements of Title 23 California Code of Regulations and the Health & Safety <br /> Code. <br /> b. A copy of the alarm history shall be printed prior to installing the new monitoring <br /> panel and made available at the time of EHD inspection. <br /> Please be aware that the facility cannot operate the UST system prior to the EMD <br /> inspection and approval. <br /> 4. If concrete will be broken near the UST or piping, a secondary containment test must be <br /> performed on the UST or piping once concrete is poured in accordance with the <br /> manufacturer's guidelines. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br />
