Waterloo Road, Stockton, CA 95205 <br />DIRECTOR <br />Donna Heran, REHS <br />PROGRAM COORDINATORS <br />Robert McClellon, REHS <br />Jeff Carruesco, RENS, RDI <br />Kasey Foley, REHS <br />SR0060381 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the application for a permit to replace the 87 grade mechanical line leak detector (LD) with a <br />new VMI LD model 99LD2000, repair the 87 grade fill spill containment by replacing the Fill <br />Swivel Adaptor 61 SALP, repair the diesel fill spill containment by replacing the Drop Tube 0 - <br />Ring and the Drain Valve O -Ring, and replace the 91 grade fill spill containment with a new <br />OPW 2100C Spill Containment on the underground storage tank (UST) system at the above <br />noted address. The above 87 grade LD has been replaced on June 30, 2010 and passed the <br />3g.p.h. leak test on the same date. <br />The permit has been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Jeff Wong at (209) 468-0335 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Spill bucket test: Verify minimum 5 -gallon capacity and pass a one hour hydrostatic <br />test (or equivalent). Proper drainage must also be verified. <br />b. Secondary containment testing on 91 grade tank and fuel line. <br />Please be aware that the facility cannot receive fuel in the 87, diesel, and 91 tanks <br />prior to the EHD inspection and approval. <br />4. If concrete will be broken near the UST and/or piping, a secondary containment test <br />must be performed on the UST and/or piping once concrete is poured in accordance <br />with the manufacturer's guidelines. <br />
