Each person installing or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />5. Contact Aris Cacapit (468-3453) to arrange the following inspection(s) (a 48-hour notice <br />is required): <br />a. If a "cold start" is needed, the EHD must witness the functionality of the <br />monitoring system to ensure that monitoring has been restored to meet the <br />requirements of California Code of Regulations Title 23 and the Health & Safety <br />Code. <br />b. Before the alarm history is cleared by the "cold start", a current printout must be <br />created and available for EHD inspection. Inspection may occur during the <br />functionality test. <br />6. If concrete is being broken near the UST, pipeline and tank integrity testing must be <br />performed by a SWRCB licensed tank tester in accordance with the manufacturer's <br />guidelines. Results must be received by the EHD within seven days of returning the <br />facility to operating condition. <br />7. An "Underground Storage Tank Certification of Installation/Modification" form (form C) <br />shall be submitted by the owner or the owners' agent [pursuant to (CCR) 2635(d)] before <br />this upgrade will be considered complete by EHD. <br />8. Permit expires 180 days from date of approval.
