Waterloo Rd., Stockton, CA 95205 <br />The San Joaquin County Environmental Health Department (EHD) has completed the <br />review of the application for permit to conduct a cold start of the monitoring panel at the <br />above noted address. The permit has been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution <br />Control District for any applicable permits and requirements. <br />2. Contact Toua Yang at (209) 468-3459 to arrange the following inspections <br />appointments (a 48-hour notice is required for inspection appointment(s): <br />a. EHD must witness the functionality of the monitoring system to ensure <br />that monitoring has been restored to meet the requirements of California <br />Code of Regulations Title 23 and the Health & Safety Code. <br />3. Any changes in information and scope of work must be submitted to EHD for <br />approval prior to commencing work. All new addenda supersede the previous <br />ones. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all <br />of the following: <br />a. One of several permissible valid, current contractor's licenses issued by <br />the Contractors State License Board (may be issued to the installation <br />company), and <br />b. A valid, current California UST Service Technician certificate issued by <br />the International Code Council (ICC), and <br />c.
