The permit has been <br />approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution <br />Control District for any applicable permits and requirements. <br />2. Contact Toua Yang at (209) 468-3459 to arrange the following inspections <br />appointments (a 48-hour notice is required for inspection appointment(s): <br />a. Spill Bucket: Verify minimum 5 -gallon capacity and pass a 60 -minute <br />Lake Test or equivalent. Proper drainage or removal must also be <br />verified. <br />b. Leak Detector Test: Leak detector must be tested to confirm that the <br />device can detect a leak of 3 gallons per hour at 10 psi. <br />c. 24 hour or equivalent lake test on secondary containment to verify <br />tightness. <br />3. Any changes in information and scope of work must be submitted to EHD for <br />approval prior to commencing work. All new addenda supersede the previous <br />ones. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all <br />of the following: <br />a. One of several permissible valid, current contractor's licenses issued by <br />the Contractors State License Board (may be issued to the installation <br />company), and <br />b. A valid, current California UST Service Technician certificate issued by <br />the International Code Council (ICC), and <br />
