In a telephone conversation on July 20, 2011, EHD was informed by the <br /> permit applicant Donlee Pump that the Incon programming was conducted on July 19, 2011 on <br /> an emergency repair basis, and the diesel repair is pending on additional investigation and <br /> purchasing of parts for the diesel dispenser. The permit has been approved with the following <br /> conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Jeff Wong at(209) 468-0335 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Positive shutdown and programming of the monitoring system must be verified to <br /> ensure that monitoring has been restored to meet the requirements of Title 23 <br /> California Code of Regulations and the Health & Safety Code. <br /> b. Diesel leak detector test: Leak detector must be tested to confirm that the device can <br /> detect a leak of 3 gallons per hour at 10 psi. <br /> 4. If concrete will be broken near the UST or piping, a secondary containment test must be <br /> performed on the UST or piping once concrete is poured in accordance with the <br /> manufacturer's guidelines. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br />
