Changes to your Business <br />Owner/Operator Identification Page, Hazardous Materials Management Plan, facility map, or <br />chemical description pages must be reported to our office within 30 days. <br />Once a year, businesses are required to certify to our office that their HMMP information is current <br />and accurate. You can use our HMMP Compliance Website, www.sjoesdata.org, to meet this <br />requirement. Please log onto the site and review your HMMP and chemical inventory and make <br />necessary changes. When all information is accurate, return to the Main Menu and click on the link <br />entitled "Office of Emergency Services Certification Form." Read the Certification Statement that <br />appears, check the box below the statement, and click on the "Submit Certification" button. The <br />website username and password below is for the facility shown above and should be kept <br />confidential: <br />Username: 7818 Password: NE21889 <br />If you do not have access to the Internet, please contact our office for a blank Certification Statement <br />and any forms you need. Please complete, sign, and date the Certification Statement and return it with <br />any updated documents to our office. <br />You are required to retain a current and accurate copy of your Hazardous Materials Management Plan <br />at your facility. If you have any questions, please contact our office at (209) 468-3969. Your <br />business must submits its certification no later than Monday, January 28, 2008.
