The following is a Minor violation - <br />Notice to Comply: #105 <br />105. UST tank forms were not on file with the EHD, and the ones retained on site are not current. Any <br />change of information on these forms must be submitted to the EHD within 30 days of the changes. <br />Immediately complete and submit a copy of the facility and tank forms to the EHD. Complete a separate <br />tank form for each tank. Submit the forms to EHD by January 6, 2013. <br />Provided Matt on site factsheet on CERS requirement and authorization form.
