Flyers #427 Cardlock Address. 3300 Waterloo, Stockon Date: Dec 1, 2011 <br />SU RY OF VIOLATIONS <br />CLASS CLAStM2TT - otice o <br />This is a Routine and Monitoring Certification. The following Class II Repeat violations #305, and Minor <br />- Notice to Comply #201 and 206 were observed on site. <br />201. Maintenance records for the response to the May 24, 2011 Diesel fuel alarm documented in the <br />June 2011 Designated Operator Monthly Report was not found on site. These records shall be kept on <br />site for at least 3 years . Immediately locate and maintain the on site and submit copies to the EHD by <br />01/01/2012. <br />206. The designated operator employee training for facility employee or employees (individuals who has/ <br />have been performing regular maintenance, visual inspection and maintains the alarm and maintenance <br />logs) was not performed . The designated operator shall train facility employees for which he or she is <br />responsible in the proper operation and maintenance of the UST system once every 12 months. The <br />training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD by 01/01/2012. <br />305 (Repeat). The Bravo box float and chain in the #2, 5 and 7 Dispenser UDC sumps failed to stop the <br />flow of product at the dispenser when tested. All monitoring equipment shall be maintained to activate <br />an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The <br />service technician adjusted the chain and verified functionality.
