Waterloo, Stockton Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> This is a report of the Routine Underground Storage Tank (UST) and Monitoring System Certification <br /> inspection conducted at the referenced facility by San Joaquin County Environmental Health Department <br /> (EHD). The following are violations observed: #21, 23d <br /> 21. About one cup of liquid was observed in the UDC sump of Dispenser#9/10. <br /> -- If water could enter into the secondary containment by precipitation or infiltration, it must be removed <br /> and disposed of properly. <br /> * Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste <br /> regulations, and manage it accordingly. Ensure that the sump is maintained free of liquid. Violation <br /> was corrected on site by Tanknology technician. <br /> 23d. The designated operator employee training for new employee Morgan Ellis hired in 2010 was not <br /> performed within 30 days of hire. <br /> -- The designated operator shall train facility employees for which he or she is responsible in the proper <br /> operation and maintenance of the UST system once every 12 months, and conduct initial graining within <br /> 30 days of the date of hire. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> * Ensure that employees have been trained by the designated operator, maintain the list on site, and <br /> submit a copy of the training records to the EHD by February 26, 2011. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD In ct Received By: Title: <br /> EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
