The service request proposed an <br /> emergency cold start of the Veeder-Root(VR)TLS 350 System,replacement of the 91 grade <br /> UST sump sensor VR-352 with a VR-208 sensor,and replacement of the ECPU2 Board of the <br /> Veeder Root TLS 350 System. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department, local planning or building department and the San <br /> Joaquin Valley Air Pollution Control District for any applicable permits. <br /> 2. Contact Jeffrey Wong(468-0335)to arrange the following inspection. A 48-hour notice <br /> is required: <br /> a. The EHD must witness the functionality of the monitoring system to ensure <br /> that monitoring has been restored to meet the requirements of California <br /> Code of Regulations Title 23 and the Health& Safety Code. All monitoring <br /> system sensors must be tested. Please be aware that the facility cannot operate <br /> before the EHD inspection and approval of this Post-Cold Start Functionality <br /> Test. <br /> 3. Any changes shall be submitted to the San Joaquin County Environmental Health <br /> Department(EHD)for approval prior to commencing work.All new addenda supersede <br /> the previous ones. <br /> 4. If concrete is broken near the UST system,a hydrostatic test by a SWRCB licensed tank <br /> tester,or a tank secondary containment test,must be performed in accordance with the <br /> manufacturer's guidelines and witnessed by EHD. <br /> 5. All components must be certified by an independent testing organization and labeled as <br /> required by the organization. (UL,ULC,ANSI,API,ASTM,etc. See California Code of <br /> Regulations(CCR),Title 23 Chapter 16 Table B for complete list). The component must <br /> bear the appropriate mark as required by the independent testing organization. <br /> 6. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid,current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br /> Page 1 of 2 <br />
