The permit <br /> has been approved with the following conditions: <br /> 1. Contact the Fire Department and San Joaquin Valley Air Pollution Control District for <br /> applicable permits. <br /> 2. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), and <br /> b. All valid, current California UST certificates issued by the International Code <br /> Council (ICC), that are applicable to the work being performed, and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested, repaired, or serviced. <br /> and <br /> d. Each person installing or testing any component must hold a certificate of training <br /> from the manufacturer of that component. <br /> 3. All components installed must be certified by an independent testing organization and <br /> bear the appropriate mark as required by the independent testing organization. <br /> 4. Contact Michelle Henry(953-7699) to arrange the following inspections appointments <br /> (a 48-hour notice is required for inspection appointments): <br /> a. EHD must verify the overfill prevention system is in compliance with CCR 2635 <br /> (b)(2).Note: Your company may choose to have EHD onsite during the overfill <br /> prevention system installation so that portions of the system do not have to be <br /> disassembled/reassembled. <br /> b. If spill bucket must be disassembled for drop tube installation it shall be tested <br /> post installation to verify a minimum 5-gallon capacity, and pass a 60-minute <br /> Lake test or equivalent. Proper drainage or removal must also be verified. <br /> 5. Complete and submit a copy of the Underground Storage Tank Certification of <br /> Installation/Modification for the 87 tank no later than 30 days after work is completed. <br /> 6. This permit expires 180 days from the approval date. <br />
