Stockton CA 95205 <br /> The San Joaquin County Environmental Health Department(EHD)has completed the <br /> review of the application for permit to install a drop tube and overfill prevention device <br /> in the regular unleaded (87)tank at the above noted address. The permit has been <br /> approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Contact Toua Yang at(209)468-3459 to arrange the following inspections <br /> appointments (a 48-hour notice is required for inspection appointments): <br /> a. EHD must verify the overfill prevention system is in compliance with <br /> CCR 2635 (b) (2). Note: Your company may choose to have EHD onsite <br /> during the overfill prevention system installation so that portions of the <br /> system do not have to be disassembled/reassembled. <br /> b. If spill bucket must be disassemble for drop tube installation it shall be <br /> tested post installation to verify a minimum 5-gallon capacity, and pass a <br /> 60-minute Lake Test or equivalent. Proper drainage or removal must also <br /> be verified. <br /> 3. Any changes in information and scope of work must be submitted to EHD for <br /> g p <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 4. All components installed must be certified by an independent testing organization <br /> and bear the appropriate mark as required by the independent testing organization. <br /> 5. Qualified installers must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board (may be issued to the installation <br /> company), and <br /> b. A valid, current UST Installation/Retrofitting certificate issued by the <br /> International Code Council (ICC), and <br />
