Box 2009
Stockton, California 95201
RE: ADDENDUM TO APPLICATION FOR PERMIT TO
CLOSE TWO UNDERGROUND STORAGE TANKS AT
PACIFIC BELL, LODI, CALIFORNIA
Dear Ms. Foley:
American Environmental Management Corporation (AEMC) has been contracted by
Pacific Bell (Pac Bell) to remove two underground storage tanks (USTs) at the Pac Bell
telephone switching facility located at 124 West Elm Street, Lodi, California. The original
application for permit to close these tanks was submitted to the San Joaquin Local Health
Department (SJLHD) on 25 September 1990.
Per your telephone conversation of 21 November 1990 with Mr. David Oliver, AEMC is
providing the SJLHD with written confirmation that we will collect soil samples beneath
every twenty linear feet of piping leading into the USTs. This will be done at the time
these two LJSTs are excavated and removed from the ground.
Enclosed with this letter is a FAX copy of the requested SJLHD Authorization To Release
form signed by Steve Wat of Pac Bell. This information should complete the application
for permit to close the two USTs.
If you have any questions concerning the above -referenced material or if we may be of any
further service, please feel free to give Mr. David Oliver or me a call at (916) 364-8872
Sincerely,
l
James F.
