One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board(may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Service Technician certificate issued by <br /> the International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested, repaired, or serviced, <br /> and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 6. Complete and submit a copy of a Underground Storage Tank Certification of <br /> Installation/Modification (Fortner Form Q to this office no later than 30 days <br /> after work is completed. <br /> 7. If concrete is broken,prior to the contractor signing off of the jobsite, a tank <br /> integrity test and a pipeline test must be performed by a State Water Resource <br /> Control Board (SWRCB) licensed tank tester. The test results must be submitted <br /> to SJCEHD within 7 days of completing this project. <br /> 8. All product installed must be certified by an independent testing organization and <br /> labeled as required by the organization. (UL, ULC, ANSI, API, ASTM, etc. See <br /> California Code of Regulations, Title 23, Chapter 16 Table B for complete list.). <br /> Pursuant to the Underwriters Laboratories Inc (UL) listings, only those products <br /> bearing the UL mark will be considered listed. Any part/product not marked with <br /> the UL label will be rejected. <br /> 9. Permit expires 180 days from date of approval.
