Elm St. Lodi, CA (SR0054794) <br /> The San Joaquin County Environmental Health Department (EHD)has completed the <br /> review of the application for permit to replace the discriminating sensor with a Veeder <br /> Root 794380-208 sensor in the transition box in the generator room, program TLS to <br /> recognize the new sensor, and certify functionality of the sensor upon completion at the <br /> above noted address. The permit has been approved with the following conditions: <br /> I. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board(may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Service Technician certificate issued by <br /> the International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested, repaired, or serviced, and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 3. Permit expires 180 days from date of approval. Note: If an extension to the <br /> expiration date is required, a written request must be submitted to this office prior <br /> to the expiration date. <br /> Page 1 of 2 <br />
