A George Koffel has been performing D. 0. inspections, and is not listed on the D. 0. agreement <br />statement currently on file with this office. Update this document and submit a copy to this office by <br />8/2/06. <br />The updated DO statement was submitted to your office by Tait Environmental Systems on 7/31/06; a copy <br />was also posted on site. <br />• Designated operator training record does not specify what topics facility employees have been trained on. <br />Ensure all employees are trained in accordance with Title 23 Underground Storage Tank regulations. <br />Submit a copy of the training records which includes topics covered) to this office by 8/19/06. <br />Please see attached training record and topics; a copy has also been posted on-site. <br />Please see attached Return to Compliance Certification.
