Elm Street, Lodi 95240 <br /> Service Request 0057633 —Jason Musial <br /> Service Request 0058584—Michael Schenone <br /> Hello Mr. Musial and Mr. Schenone, <br /> On July 22,2009, a permit application, SR 0057633,was submitted to repair/shorten the diesel spill <br /> bucket riser so that the spill bucket will hold five gallons at the above noted address. Copies of <br /> certifications from ICC California: UST Service Technician and UST Installation/Retrofitting for Dan <br /> Luck was attached to the application to perform the work. <br /> On August 28, 2009, Environmental Health Specialist Aris Cacapit was on site to witness the <br /> hydrostatic testing of the diesel spill bucket. Service technician Dan Luck informed EHS Cacapit that a <br /> new Phil-Tite spill bucket was installed in addition to the work that was permitted on the service <br /> request. The spill bucket passed the one hour hydrostatic test. The Service Request Inspection Report <br /> noted that SR 0057633 permit is incomplete, and that a permit addendum needs to be submitted of the <br /> new scope of work: the new spill bucket installation, cut sheets of the spill bucket and certification of <br /> Phil-rite manufacturer's training for Dan Luck. Mrs. Connie Mitchell of AT&T and Mr. Luck both <br /> received the report(Attached). <br /> On September 22, 2009, EHS Cacapit spoke with Mr. Musial requesting the above information. Mr. <br /> Musial said he would work on retrieving the information. <br /> On October 21, 2009, EHS Thuy Tran emailed Mr. Musial to request the certification of Phil-Tite <br /> manufacturer's training for Dan Luck. An email reply was received with an attachment of the <br /> certification. The training date for Phil-Tite EVR Phase I System was dated September 22, 2009. <br /> Immediately submit proof of Phil-rite Manufacturer's training for Dan Luck that was current at the time <br /> of work, in addition to a revised scope of work and cut sheets for the new spill bucket. If proof of <br /> manufacturer's training cannot be submitted,please indicate so and a new permit application would be <br /> required to be submitted for the new spill bucket. <br /> On October 15,2009, a permit application, SR 0058584, was submitted to remove and install a new <br /> ATG probe at the above noted address. <br /> On October 20, 2009, EHS Tran emailed Mr. Schenone asking the purpose of the ATG probe. He <br /> replied that the ATG probe will be used for inventory purpose only. Because the use of the ATG probe <br /> is used for inventory purpose only, Environmental Health Department does not regulate this use nor <br /> need to be present for the inspection or testing of the ATG probe. Payment for this permit can be <br /> refunded or applied as credit towards future permits. Please contact me to indicate your preference. <br /> 10/26/2009 <br />
