The following items need to be addressed prior to permit approval. <br />1. You need to identify the EPA # on page th ree of the application. <br />2. You need to identify the fire district and the permit number on page three of the application. <br />3. Since the date of installation is unknown, leaded gasoline constituents will be the required <br />laboratory analysis. <br />4. You need to submit a site specific health and safety plan. <br />5. You need to identify the permitted disposal site for the rinsate. <br />6. Please be aware of the sampling protocol on page seven of the application. <br />T Page ten, section four needs to be completed. <br />8. You need to choose a backfill method on the backfill excavation certificate. <br />9. Will these tanks be replaced with underground storage tanks? <br />10.
