The interstitial space shall be monitored with a monitoring system connected to an audible and visual alarm <br />system. The sensor was repaired and retested successfully during the inspection, however, since the electrical spice <br />repair was outside of the J-box, immediately contact a properly licensed, trained, and certified contractor to replace the <br />failed component with a functional component (LG 113-x listed, if applicable) under permit and inspection of the EHD. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by November 23, 2014. <br />Consent to perform the inspection, take photos and collect samples was given by Mr. Norman White. This <br />facility's designated operator is also Mr. Norman White (ICC expiration date: October 3, 2016) . The service <br />technician was Joseph Bartholdi (ICC expiration date: 8-5-2016), who had the following manufacturer's <br />certifications: VR- January 21, 2015. <br />Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br />documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br />Certification of Financial Responsibility, UST Response Plan, UST Owner/Operator: Written Agreement (if <br />applicable), UST Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator <br />Compliance. <br />ALL EHD SrATF TIME ASSOCIATED WITH FAILING TZ5 COMPLY 13Y THI!
