Contact the Fire Department and San Joaquin Valley Air Pollution Control District for <br /> applicable permits. <br /> 2. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being tested,repaired, or serviced. <br /> 3. All components installed must be certified by an independent testing organization and <br /> bear the appropriate mark as required by the independent testing organization. <br /> 4. Contact EHD to schedule the following appointments (allow 48 hour notice for all <br /> inspections): <br /> a. Corrosion protection on the fill riser. <br /> b. Verification that the new spill bucket holds a minimum of 5 gallons and passes a <br /> 60 minute lake test or equivalent after concrete is poured. <br /> 5. A tank integrity test must be performed by a third party certified tank tester and results <br /> received by this office within 7 days of completion of work. <br /> 6. Complete and submit a copy of a UST Installation Certificate of Compliance (Former <br /> Form C) for the 91 tank no later than 30 days after work is completed. <br /> 7. This permit expires 90 days from the approval date. <br /> 8. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br />
