Please <br />complete the following items within 30 days of the close of escrow [CA Health and Safety Code section <br />25284 (b) & (c)]: <br />1. Underground Storage Tanks — Tank Forms — Provide all current information,. sign and date the <br />bottom of each form. <br />2. Certification of Financial Responsibility/Letter from Chief Financial Officer: Complete these <br />forms provided. Keep a copy onsite and submit copies to this office. This document is required <br />to be submitted to this office annually. <br />3. Underground Storage Tank Monitoring and Response Plans: Complete and submit a copy for <br />approval. An approved copy will be returned. Retain an approved copy of these plans onsite for <br />future reference by employees. <br />4. Report#5021: Update all information for all programs at this site including the UST, Hazardous <br />Waste, Food programs, sign and date. Bring with all other forms to our accounting office to pay <br />for transfer fees and the $125/hr UST Consulting fee, if necessary. <br />6. If you are the UST owner, you must submit a notification form identifying the "Designated UST <br />Operator" for each facility owned. A notification form is enclosed for your use. <br />7. Also enclosed for your information is a brochure of Best Management Practices (BMP's) for <br />Retail Gasoline Outlets that you can use as part of your BMP's for your facility. <br />9. Hazardous Materials Management Plan: Contact the Hazardous Material Inspector Mike Parissi <br />at (209) 953-6213 for information on updating this plan. <br />
