Please revise the header from "Authorization for the Release of Records" to "Authorization for Representation of <br />Owner" and have Mr. Misson resign the new revision. <br />You will be an authorized representative for Mr. Misson to fill out documentations on his behalf. <br />During Owner's Consultation, we will discuss and fill out the necessary forms that need to be completed. Please bring a <br />check for $125 for the consultation. I am available next week.
