a. EHD must witness the functionality of the monitoring system to ensure that <br />monitoring has been restored to meet the requirements of California Code of <br />Regulations Title 23 and the Health & Safety Code. <br />7. If concrete is broken near the UST, pipeline and tank integrity testing must be performed <br />by a SWRCB licensed tank tester in accordance with the manufacturer's guidelines. <br />Results must be received by the EHD within seven days of returning the facility to <br />operating condition. <br />8. An "Underground Storage Tank Certification of Installation/Modification" form (form C) <br />shall be submitted by the owner or the owners' agent [pursuant to (CCR) 2635(d)] before <br />this upgrade will be considered complete by EHD. <br />9. Permit expires 180 days from date of approval.
