The test date on the <br />D.O report was August 9, 2014. The correct date is August 1, 2014. During the monthly inspection, the designated <br />operator shall check that all required testing and maintenance for the UST system have been completed and <br />document the dates they were done. Ensure that designated operators performing monthly inspections at this facility <br />are including all of the required information on the reports. <br />The designated operator failed to document all the alarms from the attached alarm history on the December 2014 <br />designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br />missing alarms include: <br />- 1-1: 87 STP Sump- November 7, 2014 Fuel Alarm <br />- L7: Dispenser 1-2- November 7, 2014 Fuel Alarm <br />During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br />that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br />documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br />monthly inspections at this facility are including all of the required information on the reports. <br />This is a repeat violation, Class II. <br />306 <br />CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br />The 87 STP sump and the 89 STP sump sensors were raised approximately three inches off the lowest point of the <br />sump and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be <br />able to detect a leak at the earliest possible opportunity. The sensors were relocated to the lowest point of the sump <br />by the service technician. Ensure that all monitoring equipment is maintained to detect a leak at the earliest <br />opportunity. This was corrected on site. <br />This is a Class II violation. <br />307 <br />CCR 2632(c) Monitoring system of the interstitial space of a double walled system not maintained. <br />The 91 annular sensor failed to activate an audible and visual alarm when tested and, therefore, the tank is not <br />monitored. The interstitial space shall be monitored with a monitoring system connected to an audible and visual <br />alarm system. The sensor was replaced and retested during the inspection. A "UST Retrofit Verification with <br />Inspector Already Onsite" form has been completed and provided to the operator and contractor.
