The information on this monitoring program are conditions of the operating permit. The permit holder must notify San Joaquin County Environmental Health Division,phone(209)468-3420 within 30 days of any changes to the monitoring procedures, unless required to obtain approval before making the change. Required by 23 CCR,Division 4,Chapter 16,Sections 2632(d)and 2641(h) . Facility Name Lawrence Livermore National Laboratory Site 300, Tank 882-D1U1 Facility Address Corral Hollow Road south of Tracy,California A. Describe the frequency of performing the monitoring: Tank Continuous Piping Continuous B. What methods and equipment,identified by name and model,will be used for performing the monitoring: Tank Universal Leak Alert Hazardous Substance Sensor(Alarm Panel Model LA-02, Liquid Probe Model LALS-1). The liquid probe is installed in the tank's annular space at the low point. Piping Universal Leak Alert Hazardous Substance Sensor(Alarm Panel Model LA-02, Liquid Probe Model LALS-1). The liquid probe is installed in the double-wall piping SUMP. C. Describe the location(s)where the monitoring will be performed(facility plot plan should be attached): On the southwest side of Building 882. D. List the name(s)and title(s)of the people responsible for performing the monitoring and/or maintaining the equipment: Person(s)designated by Don Hill,Senior Supervisor for Maintenance Mechanics. E. Reporting format for monitoring: Tank A sign-off sheet is maintained at the monitoring system control panel. The sheet is initialed at the time of completion of the weekly inspection and alarm trial. Completed sheets are maintained in the Maintenance Mechanics files in Building 875. Piping The inspection and alarm trial for the tank system includes the piping system. F. Describe the preventive maintenance schedule for the monitoring equipment. Note: Maintenance must be in accordance with the manufacturers'maintenance schedule but not less than every 12 months: At least annually,the tank interstitial space sensor and the piping sump sensor will be removed and inspected for evidence of damage or deterioration. The monitoring system monitoring manual will be used to verify the operational condition of all system capabilities. Operator will complete the form at Appendix A,Operator's Monitoring Report. Documentation will be maintained in the Building 873 Alarm Electricians files. G. Describe the training necessary for the operation of UST system,including piping, and the monitoring equipment: At the time of job assignment and not less frequently than annually thereafter, each operator and backup operator will read the monitoring system operating instruction manuals,and use the operating instruction manual to verify the operation condition of all capabilities by operating system. Each trained person will complete the form at Appendix B,Training Documentation. Documentation will be maintained in the Building 875 Maintenance Mechanics files. 12/19/94 Rev.
