Reporting Format A description of the actions taken to repair the underground storage <br /> (continued) tank and to prevent future releases [If the method involves a <br /> change as defined in Health and Safety Code, Section 25286(a), <br /> TAGG must prepare the appropriate notifications pursuant to that <br /> article.]; and <br /> • A description of how the monitoring system was reactivated after <br /> replacement or repair of the tank. <br /> The record should be submitted to TAGG within five(5)working days <br /> after the release is detected. TAGG must keep the record in the TAGG <br /> QA file for a minimum of five (5) years. <br /> A release of product to the environment which may adversely impact <br /> the environment is a reportable release. A reportable release is defined <br /> in the California Health and Safety Code, Division 20, Chapter 6.7, <br /> Section 25295 and meets any one of the following criteria: <br /> • Cleanup cannot be performed within 8 hours; <br /> • Release is not confined to the secondary containment(secondary <br /> tank and/or secondary piping); <br /> • Release deteriorates the secondary containment (secondary <br /> tank and/or secondary piping);and <br /> • Release increases the hazard of fire or explosion. <br /> Following the detection of a reportable release, the Operator must <br /> immediately notify the Environmental Analyst, who must then notify TAGG <br /> of the occurrence. <br /> A TAGG staff member must obtain verbal approval from the Division Leader <br /> of ORAD before reporting the release to the SJCPHS. The County must <br /> receive a verbal report of the release within 24 hours. Within five (5) <br /> working days after the release is detected,TAGG must submit a written <br /> report, including a copy of the Underground Storage Tank Unauthorized <br /> Release (Leak)/Contamination Report (Form #HSC 05-8/90), to the SJCPHS. <br /> This report must include other required information in accordance with 23 <br /> CCR 2652(c). <br /> Checklist For quick reference, a checklist of"Responsibilities for Tanks 879-DlUl and <br /> 879-G3Ul"is provided at the end of this document as A1212endix D. <br /> Monitoring and Response Program for 879-DlUl and 879-G3U1 10 of 14 <br />
