Contact the local fire departrnc'nt W local jurisdictions for applleable permits and <br />requirements. <br />2. Sobedule removal appointment 48 hours in advance, <br />3. In addition, the following documentation shall be submitted within 30 days of the <br />tank removal dace: <br />(a) Analytical results, mailed directly from laboratory to RM, including a chain of <br />custody and quality assurance an4 quality control (QA & QC). <br />(b) LUST Tracking Record Sheet(s) and/or Tank Hazardous Waste MSnJ 'est(s). <br />(r-,) `x', Closure Certification Form. <br />(d) Hazardous W ate Manifests for piping, rinsate, residual fuel, or waste oil receipt. <br />A site which has had UST(s) removed, shall not be considered for final closure <br />until the above items are submitted for review, <br />4. Perimiit exlpires within 90 days.
