Disposal Information: Public Health Services Tracking sheet will accompany each tank removed Som <br /> site. Tank(s) will be issued an identification number. which PHS-Ei-M's representative will note on <br /> the Tank Tracking Sheet and Contractor will affil same ID Number onto tank end using fluorescent <br /> spray paint. Contaminated tank'piping are to be transported under Hazardous Waste Ivianifest by a <br /> registered hazardous waste hauler. <br /> 3. Contractor to Provide: Combustible.Ftammabie gas detector to verify the Lower Explosive Limit <br /> (L'Z) atmosphere and oxygen level of tank prior to lining from excavation, adequate number and <br /> appropriate types) of fire cainguisher(s),barriers to secure the area as ae^c=ary to minimize traffic <br /> and pedestrian interference,and tluoresccat spray paint to affix tank(s)identification number. It shall <br /> be the project manager's responsibility for compliance with all health & safety precautions and <br /> requirements,which shall be strictly adhered to at all times during the course of the closure activities. <br /> 4 The PIIS-EHD permit shall be on site during ezmvation and removal. <br /> S. Any changes in this document must be approved by this department prior to initiating work. <br /> 6. Closure-in-Placc: If Closure-in-Place is the suggested method of abandonment; complete form <br /> EH 23 039 and submit written approval from the local Fire Department. <br /> J. Temporary CIosure: If Temporary Closure is the suggested method of abandonment; complete <br /> form EH 23 0:6 and submit written aoproval from the local Fire Department. <br /> B. The foIlowin; shall be submitted 30 days after tank removal date: <br /> a) Analytical results, mailed directly from laboratory to PHS,with a chain of c.:s.odv. <br /> b) Tank Tracking Record Sheets) and/or Hazardous Waste ;vianifest(s). <br /> c) Hazardous Waste Manifests for rinsate, residual fuel,waste oil rec-.ipt, and piping. <br /> d) Submit a compaction report as required by San Joaquin County and its incorporated City <br /> Building Departments.
