UCRL-AR-103194 <br /> (d) All monitoring programs shall include the following: <br /> (1) A written routine monitoring procedure which includes,when applicable: the <br /> frequency of performing the monitoring method,the methods and equipment to <br /> be used for performing the monitoring, the location(s)from which the <br /> monitoring will be performed, the name(s)or title(s)of the person(s) <br /> responsible for performing the monitoring and/or maintaining the equipment, <br /> and the reporting format. <br /> (2) A response plan developed by the permit applicant which demonstrates,to the <br /> satisfaction of the local agency,that any unauthorized release will be removed <br /> from the secondary container within the shortest possible time and no longer <br /> than the time consistent with the ability of the secondary container to contain the <br /> hazardous substance. The response plan shall include, but is not limited to,the <br /> following: <br /> (A) A description of the proposed methods and equipment to be used for <br /> removing the hazardous substance,including the location and availability <br /> of the required equipment,if not permanently on-site, and an equipment <br /> maintenance schedule for the equipment located on-site. <br /> (B) The name(s)or title(s)of the person(s)responsible for authorizing the <br /> work to be performed. <br /> Authority: H&SC 25299.3 <br /> Reference: H&SC 25281, 25291 <br /> A-2 Monitoring Program for New Underground Storage Tank <br />
