Disposal Information: Public Health Services Tracking sheet shall accompany each tank removed from sue. Tank(s) <br /> Will be issued an identification number which PHS-EHD's repmsentative will note on the Tank Tracking Shat. <br /> Contractor will affix same M Number onto tank end using nuorescent,spray paint Conjaminued tarkViping ue to be <br /> Vansported under Hazardous Waste Manifest by a registered ha=bz waste h u er <br /> I Contractor to : CombustibicMarnmable gas detector(w ver*the Lower Explosive limit(LEL.)atmosphere <br /> and oxygen Level of tank prim to lifting from excavation),adequate members and appropriate type(s)of fire <br /> extinguisher(s),barrios to secure the area as necessary to minimise traffic and pedestrian interference,and <br /> spray paint to afrrx tanks)identification number. h shall be the project manager's responsibility for compliance with all <br /> health tit safety regulations and requirements.which shall be strictly adhered to at all tithes during the course of the <br /> closure activities. <br /> 4 The PHS-EEID permit shaU be on site during excavation and removal. <br /> S.
