Cross out <br /> portions of the procedures submitted that will not be used on the project. <br /> Decontamination Proc dares Summary <br /> Attach additional pages as necessary to completely outline decontamination procedures to <br /> be used on site. <br /> Copy of company's standard decontamination procedures can be attached if properly <br /> cross-referenced against questions asked on summary sheet. Cross out portions of the <br /> procedures submitted that will not be used on the project. <br /> Site Access Control and Security <br /> In each box, identify specific type of material/equipment to be used, and locations where <br /> material will be used. Examples: <br /> Warning Signs - "Danger, Construction Zone -- Hard Hats Required" signs shall <br /> be posted at all entrances into the worksite. <br /> Caution Tape - Red Danger, Contaminated Area tape will be used to separate the <br /> Contamination Reduction Zone from the Clean Area. <br /> - Fencing - Yellow Polyethylene "Snow" Fencing will be used to isolate the soil <br /> pile. <br /> Plastic Sheeting Nylon filament reinforced 20 mil polyethylene sheeting will <br /> be used to cover the soil pile at the end of each days work. <br /> Emergency 1eres Summary <br /> Attach additional pages as necessary to completely outline emergency procedures planned <br /> for the site. <br /> Copy of company's standard emergency procedures can be attached if properly cross- <br /> referenced against questions asked on summary sheet. The intent herein is to ensure proper <br /> training and medical surveillance of any and all workers on-site. <br /> Attach =12ies of initial and refresher training certificates for each listed employee. <br /> Arrange certificates in same sequence as employees are listed on summary sheet. <br />
